  • Nursing home residents in one innovative care model experienced a 25 percent reduction in all-cause hospitalizations and a 40 percent reduction in potentially avoidable hospitalizations, compared to control groups.
  • model for treating depression among older adults doubled the effectiveness of treatment in primary care settings—with at least a 50 percent reduction in symptoms, compared with only 19 percent of those receiving usual care.
  • program to provide primary care for older adults in home settings saved Medicare more than $10 million—an average of $1,010 per beneficiary—while delivering higher-quality patient care.
