Health Affairs Blog: Primary Care Innovations Can Improve Alzheimer’s And Dementia Care For Patients

Health Affairs has published a blog describing JAHF-funded efforts to disseminate a successful intervention for managing Alzheimer’s and dementia care through integration of specially trained nurse practitioners with primary care teams.

In "Primary Care Innovations Can Improve Alzheimer’s And Dementia Care For Patients, Family Caregivers, And Providers," authors including JAHF Program Officer Jane Carmody discuss the University of California, Los Angeles (UCLA) Alzheimer’s and Dementia Care (ADC) program.

The blog details the support to primary care practices that the model offers by providing nurse practitioner co-management of patients with dementia, and explains how it fills a critical gap, addressing the needs of patients and family caregivers. The results of a survey of participating primary care practices and perceptions of barriers and opportunities for dissemination are discussed.
