Health Affairs has published a blog, "As Home Care Expands, Let’s Make It Safer For Everyone," which presents key findings from a project to better understand the scope of patient safety issues in the home setting.

The article discusses the recently completed Institute for Healthcare Improvement (IHI) project to better understand the extent of safety issues in home care, what infrastructure already exists and what the path forward looks like to reduce the risks for both care recipients and their caregivers. Recommendations are underscored by five guiding principles.

"Home care agencies and workers can take lessons from the hospice field about coordination of multiple caregivers, and they should be encouraged to test new models of team-based care. There are a number of programs in operation throughout the country, including Hospital at Home and the CAPABLE program."
